Capacity Building Partnership
Strengthening the delivery of the Community Connections Program, funded by the
Department for Human Services.
What is the Community Connections Program?
Community Connections is a Department of Human Services (DHS) funded program that supports people who are not eligible for mainstream programs, such as NDIS and My Aged Care, to be more involved in their community and to connect to support networks and services. Community Connections is a short-term program (up to 12 weeks) delivered through a network of Community Connections partners across South Australia. Each person is supported to establish and maintain meaningful social connections with community, supportive social networks and a sense of belonging.
Capacity Building Intent
LBI Foundation is partnering with Community Centres SA in a whole-of-state capacity building initiative.
Community Connection Program (CCP) Partners are being supported and empowered to develop the shared: (1) language, (2) knowledge, (3) skills and (4) methods to build wellbeing and trauma responsive local communities and:
- Intentionally respond (rather than react) to their clients/consumers in an intentional, growth-focused and trauma-responsive manner.
- Develop a shared intent with their clients/consumers and stakeholders around shared goals and plans.
Actions
Brokered and managed by Community Centres SA, LBI will partner with the CCP Partner agencies in a 9-12 month capacity building initiative. All work is founded upon the IMPACT Program.
This capacity building initiative includes the following key features:
- All partner agency staff having the opportunity to be trained and accredited to the Introductory Level of the IMPACT Program (click here for accreditation levels). This includes:
- Completing the One-Day IMPACT Foundations: Introduction to IMPACT and Intentional Trauma-Responsive Practice (June/July 2024).
- Completing 1-hour Coaching Modules on (1) respond to need, (2) grounded and (3) growth action planning. (October 2024 and Early 2025).
- Completing a 3-hour self-paced online training on Trauma-Responsive Practice: A Way of Thinking and Being.
- 2 x-60 minute leadership coaching sessions, founded upon embedding IMPACT content into local action (July to November 2024).
- 6 x 90-minute Community of Practice sessions, focused on taking the IMPACT content to local challenges, needs and contexts (July 2024 to mid 2025).
- All partner agency staff having access to 8 extension Specialist IMPACT Workshops across 2024-25 (July 2024 to June 2025).

How is IMPACT shaping your work?
Using the What What How® during such a busy time period has meant that I’ve been able to stay focused and get what has been essential done first. When you can’t get everything done, getting done what is important is certainly a helpful strategy! I’ve also updated our referral planning template with the What What How® structure, and we have now implemented talking through potential referrals during our intake meetings using this template as well.
Loving how versatile and easy to implement it is - it took maybe 30 minutes of work to contextualise it.
Where have you explored or applied the What-What-How in your practice since the Foundations training?
I have amended our referral template we use to do weekly case reviews to include the What What How® template, and I have started using the What What How® on myself on busy days to structure what to get done first. I also use it to structure phone conversations with participants where there is a need to actively keep a conversation focused.
